Understanding PMS and Perimenopause

Not so long ago, doctors believed that women went from having regular monthly periods to gradually stopping all menstrual cycles. Although some admit that some women have problems with the transition in between, recently the entire medical world has finally agreed that the transition period can cause serious health problems (mental and physical) for women. Doctors are finally recognizing the existence of perimenopause and the number of women who are affected each year.

One of the main reasons why perimenopause has been overlooked for so long is because of another female phenomenon known as pre-menstrual syndrome (PMS.) PMS and perimenopause can have very similar symptoms. Many doctors think that the symptoms we now know as perimenopause symptoms are actually just PMS symptoms. Since PMS symptoms tend to pass after a week (at most), doctors do not pay attention to women’s complaints.

Why do they confuse them? Perhaps because PMS and perimenopause both present symptoms such as anger, anxiety, bloating, body aches, mood swings, irritability and loss of libido. These symptoms do not seem to be a big problem to doctors, especially if a woman admits that she is used to experiencing PMS every month. The appearance of other perimenopause symptoms that first alert doctors that something may be different, symptoms such as overwhelming fatigue, hot flashes, night sweats and insomnia.

So why do any of these symptoms (whether for PMS or perimenopause) occur in the first place?

During PMS and perimenopause, women’s hormones fluctuate. People usually assume that their sex hormones fluctuate, but it’s important to point out that stress hormone levels and blood sugar hormone levels can also fluctuate. For PMS, these hormones fluctuate to prepare the body for the child’s accommodation and (later) for blood secretion. For perimenopause, hormones fluctuate because the system that prepares a woman’s body for reproduction no longer works like clockwork.

As a result of these fluctuations, the female body sends signals (symptoms) that something is happening internally. Sometimes the only symptom women will feel is that “something is wrong” and they don’t know why they feel sad or upset.

For women with PMS, symptoms usually last for a week or more. Women who are experiencing perimenopause, on the other hand, will find that they may have one or two symptoms at a time. It’s for this reason that most perimenopause experts advise women (usually 35 and over) to start recording any feelings or symptoms they may notice. Taking note of the time of the month symptoms should be noted, as well as the frequency at which certain symptoms are present can help both women and their doctors determine what is going on.

Once the doctor has determined whether or not you are experiencing perimenopause or if you have just had an acute episode of PMS, he can then prescribe a treatment plan that you can follow. Many doctors recommend mild pain relievers, such as mefenamic acid, for the body aches you may feel. Other doctors may try to give you birth control pills or progestins, to help regulate your periods and reduce PMS and perimenopause symptoms. Other doctors may still recommend hormone replacement therapy (HRT, for perimenopausal patients only.)

As the science of both PMS and perimenopause advances, however, more and more doctors are recommending more natural methods of dealing with the problem. For example, PMS has been known to decrease when a woman follows a healthy diet, exercises regularly (about 30 minutes a day), and takes up a hobby or past time that she enjoys. These three lifestyle factors can actually make PMS go away!

Because of the success of this natural treatment method among PMS patients, many doctors are now advocating natural treatments for perimenopause as well. Apart from lifestyle changes, however, herbal remedies or natural supplements are also recommended for perimenopausal women. There are many natural treatments that seem to be very effective for women, and have been shown to have fewer side effects than HRT.

The science of both PMS and perimenopause is still evolving. Now that doctors have acknowledged the presence of perimenopause, more help is coming to women. Keep in mind that if you have really bad PMS when you’re young, your chances of having severe perimenopause symptoms are high. However, regardless of which one you experience, always choose to try lifestyle changes before going for a pharmaceutical cure.
