Buttock Lift – Plastic Surgery in the Extreme Weight Loss Patient
Like the rest of the body after a lot of weight loss, the butt area is no exception. Although this is not usually at the top of the list of desired body changes, it can become a focus after other higher priority areas such as the stomach, arms and breasts are addressed.
Under the normal aging process, the back sags over time, more in some people than in others. After extreme weight loss, this sagging is greatly exaggerated. Buttock deflators create loosely hanging folds of skin that either fold over the lower buttock folds or cause the buttock to shift downward as a whole. In women, this sagging effect is usually much greater than in men because they usually have more back skin and fat to begin with.
A butt lift can be performed with a number of different methods, approaching the problem from either the top or the bottom depending on the back problem and other concurrent procedures being performed. One of the most effective methods is the upper or upper back lift which is the back of the body girth lift. Skin and fat are trimmed around the waistline above the buttock area as part of a 360 degree body lift cut. Although not as much skin can be removed in the lower back as in the front of the abdomen, the buttocks can be lifted a lot. Think of this procedure as ‘pulling up your pants’ so to speak. This butt lift can also be done alone, without the front part of the lift, but it is unusual to have a noticeably sagging butt after weight loss without an abdominal overhang as well.
Although a superior body lift or butt lift lifts sagging skin, it will not cause the butt to ‘re-plump’ or resolve the loss of volume due to weight loss. Basically, the sagging back will be fixed but it will still be quite flat. Some plastic surgeons perform ‘auto-augmentation’ at the same time using some of the skin and fat that would normally be removed with an incision. Basically using your tissue to serve as an implant. While this is a clever use of tissue flaps, it rarely adds significant volume and often doesn’t put it where it’s really needed. (lower not higher) I’m still not too enthusiastic about this approach even if it’s your own tissue. If someone ends up wanting a bigger butt, I would usually do a butt implant next time.
Another type of butt lift is from below or lower back lift. This is the ‘old’ approach which removes the skin from the lower back past the buttock fold. This is usually not helpful in cases of extreme weight loss as it pulls the butt down further in cutting the skin. This pulls in the wrong direction for most patients. In cases of mild buttock sagging without significant volume loss, this type of butt lift may be acceptable.
